This month's selection:
My question concerns a pediatric acute care setting and CPT codes 99468, 99471, and 99475, Initial in-patient neonatal critical care. When a patient transfers from the neonatal ICU (NICU) or children’s ICU (CICU) to a regular medical floor but returns seven days later for a complication, new problem, or worsening problem, can the provider receiving the patient in the NICU or the CICU charge an initial critical care again?

Editor's E-Note
Through informal conversations with coders, I’ve learned that most are anxious to get ICD-10 up and running. The delay was met more with disappointment than relief.

Not Confuse

By George Schwend and Brian Levy, MD

The sheer volume of diagnosis codes found in ICD-10-CM—roughly 68,000—often leads to the mistaken perception that ICD-10 will create more confusion in the healthcare industry than the current 13,000 ICD-9-CM codes. The key is to ensure that provider documentation completely and accurately maps to codes with precise descriptors rather than those that are unspecified.

Considerations for ICD-10 Preparation
By David Van Doren, RHIA, CCS, RAC-CT

With the government’s one-year delay in the transition deadline for ICD-10, medical organizations have an opportunity to apply a focused effort toward enhancing overall medical documentation to minimize the impact of the change on their reimbursements. With significantly more codes than its predecessor and, more importantly, an increased level of procedure specificity, ICD-10 will require a greater deal of documentation from the clinician to ensure a smooth transition.
